Job Description
Cardiology Outpatient Advanced Practitioner opportunity in the Hartford CT region.
Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology Heart Failure team at Saint Francis Hospital and Medical Center.
RESPONSIBILITIES:
- See outpatients in various Heart Failure Centers throughout the region including St. Francis Hospital and Medical Center in Hartford. One site per day. Training begins at SFHMC.
- Provide transitional care management
- Offer guideline directed medical therapy optimization
- Perform advanced heart failure evaluations
- Provide same day access care
The successful candidate will:
- Interested in heart failure care
- Have Cardiology/Heart Failure experience (preferred but not required)
- Hold an Advanced Practice Nurse or Physician Assistant CT state license.
