Now Hiring: Care Coordinator - Children's Long-Term Support (CLTS) Program Walworth County, WI Full-Time | M-F, First Shift | Hybrid Work Option Make a difference in the lives of children and families! Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County ! The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services with compassion and care.
