Apex Systems is hiring a Care Navigator for a large Healthcare client.

Location: Sacramento, California (100% Onsite)

5 months contract with potential to extend or convert.

Hours: Monday - Friday, 8:00 AM to 10:00 PM

Proposed Shifts: 8:00 AM to 5:00 PM + 2:00 PM to 10:00 PM

Role Overview

We are seeking a Care Navigator for an onsite role in Sacramento. This position involves working within Emergency Department and Inpatient settings to support patient care coordination. The ideal candidate will possess relevant experience and skills to effectively navigate healthcare systems and assist the communities we serve.

Key Responsibilities

Member Engagement

  • Identify and prioritize patients from real-time census lists
  • Engage patients in person during care visits
  • Participate in team huddles and workflows as part of the care team
Care Coordination & Navigation
  • Help enroll patients in Enhanced Care Management (ECM)
  • Connect patients to services such as:
    • Transitional Care Services (TCS)
    • Complex Case Management (CCM)
    • Social Work and financial assistance
    • Assess social needs and link patients to community resources
Benefits Support
  • Assist dual-eligible members with Medicare/Medi-Cal alignment
  • Coordinate with internal teams for benefits and coverage questions
  • Help non-members connect with their managed care plans
Follow-Up & Operations
  • Conduct post-discharge follow-up calls
  • Document interactions and support pilot program improvements
  • Collaborate to refine workflows and engagement strategies
Required Qualifications

Education: A high school diploma is required.

Experience: 3-5 years of relevant experience is required in the following:
  • Medi-Cal, Medicare, or dual populations
  • Hospital/ED environments and care management
Technical Skills: Proficiency with Microsoft Word is required.
  • Strong communication and coordination skills
  • Ability to work in fast-paced, evolving settings
Preferred Qualifications
  • A bachelor's degree in social work, Psychology, Public Health, Human Services, Sociology, Gerontology, or a related field.
  • Bilingual and bicultural skills that are reflective of the communities served.
