Requirements:
- CNA Certificate
- Valid driver's license or Passport or Green Card and SSN Card
- COVID Vaccination Card
- MMRV Results (Mumps, Measles, Rubella, and Varicella)
- Physical Assessment
- CPR
- Must be immune to Hep B
