Dialysis Social Worker

Satellite Healthcare, Inc. seeks a compassionate Dialysis Social Worker to support patients living with kidney disease. In this role, you will provide psychosocial assessments, care planning, counseling, and crisis intervention for in-center and home dialysis patients and families. You will coordinate community resources, advocate for patients, and collaborate closely with nurses, physicians, and dietitians to improve quality of life and treatment adherence in a mission-driven, team-based environment with strong training and growth opportunities.
