Transition & Intake Director

The Transition & Intake Director at Life Unlimited will play a pivotal role in ensuring that adults with developmental disabilities receive comprehensive and personalized support services. The Transition & Intake Director is responsible for supervising Service Navigators (SN's), and ensure the department guides individuals through the agency's array of services, facilitating access to resources, and fostering a supportive environment that empowers individuals to achieve their goals and lead fulfilling lives. Additionally, the role involves establishing and nurturing connections with local community partners to promote the agency's mission and raise community awareness of that mission.

The Transition & Intake Director is responsible for:

• Ensuring exceptional quality in documentation and reporting
• Establishing and maintaining an intake process and corresponding documentation within the agency
• Monitoring utilization of intake process documentation by SN's
• Leading program growth, expansion, and improved efficiencies
• Providing supervision and guidance to team members within the Service Navigation Department
• Fostering strong communication within and between all programs and the overall organization

The position requires good communication skills both orally and in written form, ability to use computer and other electronic equipment, self-motivation, initiative and independence to work without direct supervision, flexibility, reliability, dependability, and the ability to manage and embrace change.

Qualifications include a Bachelor's Degree in a Human Service Field preferred, minimum of 5 year's management experience with one (1) of those years working with individuals with developmental disabilities strongly preferred, and obtaining and maintaining training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, TOOLS and Crisis Intervention.
