Primary Responsibilities:
Provide comprehensive primary care to a panel of assigned adult patients through the Patient Aligned Care Team (PACT) delivery model
Interface with non-VA providers (DoD and Community) for transitions of care and collaborate with VA providers for seamless coordination
Achieve clinical outcome targets in access, quality, patient safety, efficiency, process improvement, and satisfaction
Prescribe medications and place specialty consults utilizing the electronic medical record system
Uphold professional standards, VA core values, and a commitment to service excellence
Complete clinical reminders for chronic disease management and preventive care, including medication reconciliations
Ensure timely entry of progress notes, encounters, secure messages, and responses to view alert notifications
Communicate all test results, particularly abnormal results, quickly and appropriately to patients
Respond promptly to internal communications, such as TEAMS messages and Outlook emails, and complete all patient forms per VA standards
Participate in peer reviews, chart reviews, root cause analyses, and other quality assurance activities as required
Collaborate with a multidisciplinary team, including HPACT Psychologist, LCSW, RN, psychology trainees, and work closely with staff from US Vets, HUD VASH programs, GPD, and HCHV
Conduct outreach as needed, including visits to shelters, encampments, and veteran homes

Salary: Competitive and commensurate with experience - $250,000 - $315,000
