Geriatric Chronic Condition Management in a value-based care model. Mon-Fri, no weekends/eves. Spend 30-45 min with your patients, get charting done during the work-day with Athena and AI Scribe.

Responsibilities:

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs. Manage chronic conditions of assigned patient panel through 30-45 min patient visits. Achieve quality outcomes meeting HEDIS/STAR measures.

Qualifications:

  • Licensed as a NP in the State where HarmonyCares Medical Group office located
  • Federal DEA License/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS Certification
  • Board Certification
  • Must maintain a valid driver’s license and good driving record
