Graduate of a medical/osteopathic medical school is required. Demonstrated competency as a physician by completion of an appropriate residency program; board certified or board eligible. Licensed or eligible for State of Iowa physician license; Iowa Controlled Substance Certificate and Federal DEA Certificate.
