Job Duties & Responsibilities
Conduct annual wellness exams and manage preventive, acute, and chronic care across a diverse patient panel
Provide comprehensive primary care for patients across all age groups and life stages
Manage chronic conditions including diabetes, hypertension, hyperlipidemia, and related comorbidities
Support managed care initiatives and quality reporting programs
Document patient encounters accurately and efficiently using EMR and voice dictation tools
Collaborate with clinical support staff and in-house specialists for coordinated, patient-centered care
Provide patient education on disease prevention, chronic disease self-management, and healthy lifestyle behaviors
Participate in practice-wide quality improvement and clinical operations initiatives
Build long-term, trust-based relationships with patients and their families

Required Qualifications
MD or DO degree from an accredited institution
Board Certified or Board Eligible in Family Medicine
Active Arizona medical license or ability to obtain prior to start
Current DEA registration
Ability to obtain clinical privileges as applicable
Strong patient communication and interpersonal skills

Experience in outpatient primary care or private practice setting
Familiarity with managed care, quality metrics, and value-based care programs
