Provide supervision and oversight for community-based health and wellness programs that promote healthy lifestyles, physical activity, and disease prevention for citizens of the Muscogee (Creek) Nation.
Minimum Qualifications
Education A Bachelor's Degree in Health and Human Performance, Health Promotion, Physical Education, Exercise Physiology, or related field is required. Master's Degree in related field preferred.
Experience Five (5) years relevant experience in a related field.
Licenses & Certification DPP Healthy Lifestyles Coach preferred, Community Health Education Specialist (CHES) preferred, and a current State Educator License is preferred.
Knowledge & Skills
- Knowledge in health and wellness by utilizing physical activity and the role it plays in the prevention and treatment of diabetes.
- Broad based knowledge of education principles as related to health promotion.
- Knowledge of ADA, IHS, MCNDH and Diabetes Program guidelines, regulations, policies and procedures as related to health and wellness in diabetes.
- Ability to supervise and support assigned staff, including training, scheduling, and monitoring work performance.
- Skill in communicating with community members of all ages and in a variety of community program settings
- Ability to coordinate, plan and schedule age-appropriate program activities within communities
- Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals, and the general public both orally and in writing
- Basic knowledge of data management including community assessment, gathering, and reporting of pre-specified data related to program evaluation.
- Ability to utilize a personal computer in a Windows environment.
- Ability to maintain a confident professional demeanor.
Job Summary
Provide supervision and oversight for community-based health and wellness programs that promote healthy lifestyles, physical activity, and disease prevention for citizens of the Muscogee (Creek) Nation. This position develops, coordinates, and evaluates wellness initiatives; supervises assigned staff and program operations; and fosters community engagement to improve health outcomes and enhance the quality of life within assigned clinic communities.
Work Environment
Work is performed in an office and clinical environment, primarily in the Local community but also in various facilities located in communities throughout the Muscogee (Creek) Nation Health System designated service area. Travel is required in performing position duties.
Physical Demands
The work requires the demands of normal office and working within MCNHS clinics and community facilities throughout the MCN. Driving a GSA vehicle to the various work facilities is required. Must have a valid driver's license and be insurable.
Essential Functions
Satisfactory job performance will be determined by successful execution of the following:
- Assess MCN communities to identify assets and resources and needs to increase the level of physical activity of individuals of all age groups.
- Target specific community groups for intervention.
- Develop and implement community health promotion and wellness programs for targeted community groups.
- Facilitate communities in developing and providing community-specified program activities.
- Formalize all programs and activities according to MCN Public Health Program Guidelines and as adapted by the local community.
- Facilitate Tribal level support and recognition of persons and communities that increase their physical activities through program participation.
- Establish measurable objectives with which to evaluate all programs and activities and report results within specified Program guidelines.
- Work with the Health Promotion & Wellness Director and Diabetes Program Manager to maintain ADA and/or IHS Recognition of the MCN Diabetes Program.
- Assist the Health Promotion and Wellness Director with program planning, implementation, evaluation, and continuous improvement of health and wellness initiatives.
- Participate in Health Promotion team meetings to specify delivery of patient care and coordinate community program activities with the local community.
- Participate in case management and team meetings to coordinate activities, education, and integration into the patient care plan.
- Assist communities and MCN in obtaining grants for projects, etc. that promote health and wellness through activity.
- Supervise assigned staff and performance management to ensure effective delivery of Health Promotions programs.
- Maintains professional licensure/certifications.
- Regular attendance is required.
- Performs other duties as required.
