Part Time position. Lexington region./pulliProvides specific aspects of care under the direction of the clinical leader./liliReviews care plan and records visit notes. Documents care as well as significant findings./liliReports concerns or changes to leadership./liliAssists patients with promoting functional ability and maintaining a comfortable home environment./liliFollows the established plan of care at all times./liliAssists with or provides basic personal care and hygiene to patients./liliProvides bedside care in a variety of settings in accordance to Home Care/Hospice policies and procedures and in accordance to the Aide care plan./liliAssists patients with daily living, personal hygiene (including baths in tubs, shower, basin, bed, shampoos, nail care as prescribed on Aide care plan), ambulation and prescribed exercise./liliAssists patient with toileting, including use of bedpans and bedside commodes, and provides proper skin care to incontinent patients./liliAssists with oral or topical medications that the patient can normally self-administer as directed by the patients careplan./liliMeasures fluid intake and output as indicated in the care plan./liliReviews care plan and asks leaders any questions prior to service. Records visit notes.
