Responsibilities
- Helps to improve or minimize residual physical disabilities of patients assigned to PTA.
- Providing treatment as ordered by the attending physician and under the supervision of a licensed Physical Therapist.
- Consults with Physical Therapist before any change in treatment.
- Educates and instructs patients and family members in home programs for balance and gait training.
- Observing, recording, and reporting information on the patient's condition to the attending physician, supervising Physical Therapist, case manager, and in the patient's health record.
- Teaching and supervising other healthcare personnel when appropriate.
- Evaluating the home environment and making appropriate recommendations.
Qualifications
- Licensed/registered to practice as a PTA in the state of practice.
- Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician's health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
- Has the ability to read and follow written instructions and accurately document care provided.
- Is self-directing and has the ability to work with little direct supervision.
- Possesses and maintains current CPR certification.
- Is flexible and cooperative in fulfilling the role obligations.
- Demonstrates tact, patience, and good personal hygiene.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $45 to $70 visit commensurate with experience and depending on type of visit. The compensation reflected on this posting is an estimate of yearly compensation.
