Reporting to the MD of Hospice, the LVN provides skilled nursing care under the supervision of a Registered Nurse, delivering compassionate hospice services in patients' homes and care facilities. You'll support care plan implementation, collaborate with the interdisciplinary team, and help ensure every patient receives high-quality, patient-centered care.
What You'll Do
Key Responsibilities
- Deliver skilled nursing care following directions of an RN & the patient's plan of care
- Assist the RN with care coordination and interdisciplinary collaboration
- Educate patients and families on care plans and self-care
- Monitor patient conditions and report changes promptly
- Maintain accurate clinical documentation
- Support infection control and regulatory compliance
- Assist with end-of-life care and patient advocacy
- Promote quality care and agency values
- Perform other duties as assigned
What We're Looking For
Qualifications
- Graduate of an accredited Vocational Nursing program
- Active Texas LVN license and current CPR certification
- 2+ years of clinical LVN experience required; Hospice or home health experience preferred
- Valid Texas driver's license, reliable transportation, and current auto insurance
Additional Requirements
- Ability to stand, walk, lift, up to 50 lbs and safely transfer patients as needed
- Ability to travel locally throughout the assigned service area
- Compassion, professionalism, adaptability & ability to work independently and as part of team
- Strong communication, team-work, critical thinking & problem-solving skills
- Flexible schedule with PRN emergency call participation
- Ability to work effectively in fast-paced, emotionally demanding environments
- Must pass a background check and provide TB and Hepatitis documentation
