Hybrid - Bilingual RN Care Manager - MLTC - Registered Nurse

Compass Healthcare Consulting & Placement is conducting a search for a Hybrid Remote and Field RN Care Manager for a Managed Care Plan servicing New York. The RN Care Manager position works as a Partial Remote Case Manager, working primarily from home, with 1 day of Field Case Management visits per week, in designated area. Bilingual Chinese, Spanish or Russian Registered Nurses needed for Brooklyn/Staten Island, Bronx (Spanish preferred), Manhattan (Spanish preferred), and Queens (Russian). Bilingual RN with MLTC experience encouraged to apply!

Qualified candidates will be able to work from their home 4 days per week and will make occasional Field visits, approx.1 day per week in the field. Qualified candidates will have prior RN Case Management experience within Managed Care, Home Care or Hospital Case Management, bilingual Spanish, Chinese and Bengali language ability required in some areas. Qualified candidates will have a NYS RN License or NYS NP, and prior RN MLTC Case Management experience.

Patient visits area available in Lower Manhattan, the Bronx/Westchester, Brooklyn/Staten Island and Queens/Nassau, NY. Bilingual language ability required.

Hybrid Remote & Field - RN Care Manager Responsibilities:

Reporting to the Supervisor of Care Management, this position will provide telephonic and in-person case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs.managed care nurse, managed care rn, nursing, rn, nurse, care management

Qualifications:

• NYS RN license required
• CCM preferred
• At least 2+ years of RN Case Management experience in managed care MLTC
• Ability to make patient visits up to 2 days per week, ability to work remote from home the remaining days
• Reliable transportation
• Covid19-Vaccination
• Bilingual Chinese, Spanish or Bengali in demand!
• Prior experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations
• Will consider RN's with MLTC or CHHA Home Health Care Management experience

Excellent Salary $110,000 to $115,000, based on prior experience & great Benefits! Plus $10,000 Sign-On Bonus!
