Licensed Practical Nurse

CarePartners is seeking a Licensed Practical Nurse to deliver compassionate, high-quality nursing care to patients in their homes. In this role, you will provide skilled interventions, monitor patient status, administer medications and treatments, and support individualized care plans. You’ll educate patients and families, promote safety and independence, and document care using EMR systems. Working within a collaborative, patient-centered culture, you will partner with RNs, therapists, and caregivers to improve outcomes and quality of life while growing professionally through ongoing training and support.
