Licensed Practical Nurse - Home Health

CarePartners is seeking a Licensed Practical Nurse - Home Health to provide high-quality, patient-centered care in the McDowell area. In this role, you will deliver skilled nursing services in patients’ homes, support care coordination, and collaborate closely with an interdisciplinary team to improve community health outcomes. Responsibilities include patient assessments, medication administration, care plan implementation, and patient/family education.
