KeyResponsibilities

• ProvidedirectpatientcarewithintheLPNscopeofpractice.

• Assistwithpatientadmissions,discharges,assessments,andcareplanning.

• MonitorpatientconditionsandpromptlycommunicatechangestotheRN.

• Participateinbedsideshiftreports,patientrounding,andinterdisciplinarycollaboration.

• Reinforcepatientandfamilyeducation.

• Delegateandsupportpatientcareactivitiesforunlicensedassistivepersonnelasappropriate.

• Maintainaccuratedocumentationandfollowhospitalpoliciesandprocedures.

• Completerequiredcompetencies,certifications,andongoingeducation.

• Contributetoqualityimprovementinitiativesanddepartmentgoals.

Qualifications

• GraduateofanaccreditedPracticalNursingprogram.

• CurrentLPNlicenseinthestateofpractice.

• NewgraduatesandexperiencedLPNsencouragedtoapply.

• Strongcommunication,teamwork,andpatientcareskills.

Certifications

• BLS(AmericanHeartAssociation)requiredwithin30daysofhire.

• IVCertificationrequireduponhireorwithin3monthsofhire,basedondepartmentrequirements.

• Additionalcertifications(ACLS,PALS,PEARS,S.T.A.B.L.E.,orotherunit-specificcertifications)mayberequiredbasedonassignment.
