Satellite Healthcare seeks a Medical Social Worker to provide compassionate psychosocial care to individuals living with chronic kidney disease. In this patient-first role, you will complete psychosocial assessments, deliver counseling and crisis intervention, coordinate resources, and support dialysis and home therapy patients in navigating treatment and lifestyle changes. You will collaborate closely with nephrologists and an interdisciplinary care team, contribute to care planning and education, and advance a mission-driven culture focused on improving quality of life for people with kidney disease.
