  • Responsible for transportation and accompaniment of participant to medical sites

  • Perform participants medical appointment scheduling/facilitation and transportation

  • File P.O's, control sheets and PRN sheets monthly after LPN gives them to the Program Nurse

  • Performs delivery of medications, ER packets to the residences and written updates to the LPN when needed

  • Check vehicles every Monday and report any damage or issues to the proper supervisor

  • Attend Nursing meeting

  • Perform any med errands, glasses repair

  • Purchase nursing supplies

  • Perform all office duties on a regular basis

  • Transport participants to ER

  • Maintain medical records

  • Follow up with CM, RS and Program Nurses regarding appointments

  • Perform ETPs and Prior Auth's

  • Send communications as needed

  • Assist Program Nurse with destroying meds or returning meds to the pharmacy

  • Obtain verbal consent regarding ROI from guardians before and after appointments

  • Obtain insurance information from Case Managers

  • Communicate information on all upcoming appointments

  • Maintain updated information in ER packets

  • Understands and ensures adherence to company policies and procedures; also ensure compliance with relevant external regulatory statutes and standards.

  • Provide leadership and supervision to staff in the residence and community while providing direct services to individuals we serve.

  • Attend training for individuals served to assure their objectives, and company goals are achieved.

  • Participate in community outings, house orientation to all new employees, serve as a role model to staff assuring that documentation is completed accurately and timely.

  • Review individuals served progress, coordinate, and implement shift objectives.

  • Provide for the delivery of services such as skills training, job coaching, behavior management according to the Individual Plan (IP) and/or treatment team.

  • Assumes immediate leadership role in implementing personal intervention, active treatment, and crisis prevention as needs arise during shift/while on community outings.

  • Replaces, reassigns, or cancels staffing assignments as needed.

  • Facilitates residential staff problem solving, provides direct residential staff supervision, and initiates residential staff corrective action interventions, as required.

  • Coordinates and completes participant and staff accident/injury/incident report investigation forms for incidents that occur during shift.

  • Provides coaching and leadership to LSTs to ensure that the therapeutic goals are met and that programs are facilitated in a respectful manner.

  • Performs other delegated duties as assigned


Qualifications:



  • High School Diploma or equivalent

  • Two years experience in providing direct services in the human services field.

  • Current driver's license in good standing

  • Current CPR/First Aid Certification as required.

  • Acute attention to detail and ability to problem-solve.

  • A reliable, responsible attitude and a compassionate approach.

  • Travel required as needed


Physical Requirements:



  • Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
