Outpatient Clinic Registered Nurse

Community Care Physicians, a physician-owned multispecialty medical group in Latham, NY, is seeking a Registered Nurse to deliver patient-centered care in our outpatient practices. The RN will perform assessments, administer medications, coordinate care plans, and provide education to patients and families across primary and specialty services. Working closely with physicians and support staff, this role supports quality improvement, population health initiatives, and a collaborative, team-based environment that promotes work–life balance and long-term relationships with patients and colleagues.
