Physical Therapist - Travel Contract
Apply NowJoin a Home Health team in Solon, OH as a traveling Physical Therapist. Provide evaluations, interventions, and patient education for neuromuscular, neurological, and orthopedic conditions across the home setting. Collaborate with interdisciplinary clinicians, document Plans of Care and progress, instruct patients and families in safe transfers and equipment use, and adapt rehabilitative devices for home limitations. Participate in infection control practices and accept clinical assignments consistent with education and competence. Reliable transportation and ability to travel locally are required.
Minimum Qualifications
- Current State Physical Therapist license required for Ohio prior to start.
- Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an APTA‑approved program.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and vehicle insurance required.
- 1–2 years experience as a Physical Therapist in acute care or rehabilitation settings required.
- Experience with HCHB, OASIS, and home health documentation systems required.
- Ability to perform heavy lifting, bending, pushing, pulling, prolonged standing and walking.
- Ability to work a flexible schedule and travel locally. Floating to nearby branch coverage may be required.
Preferred Qualifications
- Previous home care experience preferred.
Assignment Details
- On-call required if requested; on-call needs are subject to change based on staff and branch needs.
- Expected weekly productivity: approximately 20–30 hours; productivity units referenced at 20–30 (units guidance from client).
- Primary service areas include Cuyahoga, Summit, Stark, Lake, and Medina counties with travel between Akron and Cleveland markets.
