Minimum Requirements
- Master's degree in Physical Therapy required; Bachelor’s or Doctoral degree acceptable if meeting state licensure requirements.
- Active Kentucky Physical Therapy license required to start the assignment.
- Previous home health experience required.
- Experience using HCHB electronic medical record required.
- BLS certification from AHA or ARC may be required.
- Professional certification required as noted by state licensing.
Preferred Qualifications
- Prior experience working for LHC or similar home health organizations preferred.
Assignment Details
- Shift hours: Monday through Friday, approximately 8:00 AM to 4:30 PM.
- Weekly hour guarantee: 32 hours.
- Coverage area includes Elizabethtown, Hardin County and Hardinsburg, Meade County.
- Mileage reimbursement policy: reimbursed between visits from first patient to last patient, not to first or from last.
- No weekend or on-call requirement.
- May be required to float within the coverage area.
