Physical Therapist — Home Health — Camden, SC


Seeking a travel Physical Therapist to provide evaluations and skilled therapeutic services in a home health setting. The clinician will deliver treatment according to physicians' orders, document patient progress, and communicate changes in condition. The role includes supervision and evaluation of Physical Therapy Assistants and Home Health Aides as assigned. Reliable transportation, a valid driver’s license, and agency-required liability insurance are required for home visits.


Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body.
  • Current, active, and unencumbered SC physical therapy license required to start the assignment.
  • Current BLS/CPR certification from AHA or ARC.
  • Familiarity with electronic documentation and basic computer navigation, including email and online training systems.
  • Must have reliable transportation, current driver’s license, and agency-required liability insurance for home visits.
  • Experience providing physical therapy in home health, outpatient, or similar settings preferred.
  • Experience supervising PTAs or home health aides preferred.


Preferred Qualifications

  • 2–3 years of clinical physical therapy experience preferred.
  • Experience with care coordination and developing home-based treatment plans preferred.


Assignment Details

  • Shift: Evenings, 5x8 schedule.
  • Must perform home visits; clinician will travel within the service area and may be required to float between agency service areas as needed.

Impacting Patient Care

Provide skilled physical therapy in the patient’s home according to the physician’s plan of care. Participate in plan development, in-service education, and discharge planning. Perform documentation and reporting of patient status and collaborate with interdisciplinary team members to support safe, effective care.
