Perform patient evaluations and deliver therapeutic services according to physician orders. Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Responsibilities include evaluating home environments, developing and updating plans of care, documenting patient condition, educating patients and family members, and participating in in-service education programs.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body.
- Current, unencumbered Physical Therapist license in the state of South Carolina required prior to start.
- Current CPR certification.
- Reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.
- Familiarity with basic computer functions including email, internet navigation, and electronic documentation systems.
- Experience in home health or providing care in home environments is required.
- State-specific compliance: ability to meet applicable state regulations for home health therapy services as required.
- Preferred: 2-3 years of physical therapy experience.
Assignment Details
- Evenings shift, 5 shifts per week, 8 hours per shift.

Impacting Patient Care
Provide skilled physical therapy in accordance with the physician’s plan of care. Deliver patient-centered services that improve function, support safe home discharge, and coordinate care with interdisciplinary team members. Maintain documentation and compliance with state and agency policies. Supervise and mentor therapy assistants and support staff as appropriate.
