Minimum Requirements
- Doctoral Degree in Physical Therapy (DPT) from an accredited program approved by the APTA.
- Active Washington State Physical Therapy license required to start the assignment.
- CPR certification required.
- BLS certification from AHA or ARC may be required.
- Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and follow-up visits required.
- Experience using Homecare Homebase (HCHB) EMR preferred.
- Ability to cover a 50-mile radius for patient visits.
- Willingness to follow a business casual dress code.
- Minimum education requirement: Doctoral.
Assignment Details
- Facility type: Home Health.
- Shift name: Days.
- Shift schedule: Days 0800-1600.
- Workweek: 5x8, 40 hours per week guarantee.
- On call/weekend requirement: No.
- Mileage reimbursed between visits from first patient to last patient.
- Coverage area: up to a 50 mile radius; candidate should be prepared to travel between patient homes within that area.
