Minimum Requirements
- Graduate of an accredited physical therapy program (Bachelor’s, Master’s, or Doctoral degree) recognized by the APTA or equivalent accrediting bodies.
- Current, unencumbered license to practice physical therapy in the state of assignment.
- 2-3 years of physical therapy experience preferred. (Preferred)
- Current CPR/BLS certification from AHA or ARC.
- Experience delivering skilled physical therapy in home health or community settings, including patient evaluations, plan-of-care implementation, and discharge planning.
- Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
- Familiarity with documentation systems and basic computer skills for electronic health records, e-mail, and online training platforms.
- Reliable transportation, valid driver’s license, and required agency liability coverage for home visits.
