Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapy license with no restrictions required to start the assignment.
  • Minimum of one year of Physical Therapy clinical experience required.
  • Basic Life Support (BLS) certification required.
  • Experience with home health documentation and workflows required, including HCHB and OASIS preferred.
  • Ability to perform start of care, resumption of care, regular visits, and recertification visits in the home health setting.
  • Must be able to travel within assigned coverage counties (Clark, Crawford, Floyd, Harrison, Jefferson, Scott, Washington).
  • Minimum education requirement: Bachelor‘s degree.
  • Preferred: HCHB experience and OASIS experience.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday–Friday, 8:00 AM–4:00 PM.
  • Weekly Hours: 40 hours guaranteed.
  • On-call/Weekend Requirement: One weekend per month.
  • Mileage reimbursed between visits from first patient to last patient.
  • Primary responsibilities: HCHB and OASIS documentation, start of care, resumption of care, regular visits, and recertification visits.
