Physical Therapist - Travel Contract

You will help patients recover from injuries and manage chronic conditions while working across a rural territory that includes multiple small towns and lake communities. Expect to coordinate care in patients‘ homes and collaborate with interdisciplinary teams to support safe functional outcomes.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
- Active Wisconsin physical therapy license required to start the assignment.
- At least 12 months of home health physical therapy experience within the past 3 years. (Client requirement.)
- BLS certification from AHA or ARC may be required.
- Experience with HCHB electronic medical record preferred.
Preferred Qualifications
- Previous travel or agency home health experience.
- Experience managing a rural caseload and traveling between patient homes.
Assignment Details
- Shift Hours: 8am-5pm.
- Schedule: Monday through Friday.
- Territory includes Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O Lakes, and Phelps.
- No on-call required.
- May require travel within the assigned territory between patient homes.
