Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist team supporting a skilled nursing facility in Ada, OK, where you will help patients recover from injuries and manage chronic conditions while experiencing a new location. Providers will evaluate, treat, document, and coordinate care following regulatory and clinical practice standards.


Minimum Qualifications

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Oklahoma physical therapy license required prior to start of assignment.
  • Minimum 1 year of PT clinical experience required. New graduates considered only if they have Level II clinical experience rather than solely Level I observation.
  • Experience in long term care (LTC) or skilled nursing/rehabilitation settings required.
  • Ability to perform standardized assessments and provide interventions including manual techniques, therapeutic exercise, ambulation training, modalities, use of assistive devices, and wound care as indicated.
  • Ability to document clinical care and treatment minutes in electronic medical record systems per regulatory requirements.
  • Ability to supervise and support physical therapist assistants, rehabilitation aides, and students.
  • Adherence to APTA and state practice acts, professional code of conduct, and ethics.
  • BLS certification may be required by facility and should be obtained if requested.


Preferred Qualifications

  • Prior travel therapy experience.
  • Experience obtaining DME and supplies per regulatory and clinical practice requirements.


Assignment Details

  • Shift hours: 8-hour shifts.
  • General weekly workweek: 35 GWW (guaranteed work week).
  • Shift pattern: 5x8 schedule.
  • No weekend requirement noted.

Impacting Patient Care

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wounds. You will develop and adjust individualized plans of care, evaluate treatment outcomes, provide patient and caregiver education, and participate in interdisciplinary meetings and required trainings. Accurate clinical documentation and timely communication with the care team are required. You will provide discharge summaries and make referrals when appropriate.
