Minimum Requirements

  • Doctoral Degree in Physical Therapy required. (Client requires Doctorate Physical Therapy).
  • Active Oregon Physical Therapy license required prior to start of assignment.
  • BLS certification required or may be required from AHA or ARC.
  • Home Health experience preferred. (Client notes Home Health experience as a differentiator).
  • Experience with HomeCare Homebase EMR preferred.
  • Experience performing PT visits including SOC, evaluations, recertifications, ROC, and supervisory visits preferred.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Days: Monday through Friday.
  • Shift Hours: 0800–1600 (client-provided schedule lists Mon–Fri 0800–1630 in job notes; assignment details list Days 0800–1600).
  • Weekly Hours: 40 (weekly guarantee noted by client).
  • Coverage Area: Josephine County.
  • Mileage reimbursed between patient visits from first to last patient.
  • Required responsibilities: Start of Care (SOC), PT evaluations, recertifications, Return of Care (ROC), and supervisory visits.
