Minimum Requirements

  • Associate, Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program.
  • Active Ohio Physical Therapist license required prior to start.
  • Minimum one year of home health physical therapy experience required.
  • Experience with HCHB (HomeCare HomeBase) documentation required.
  • BLS certification from AHA or ARC may be required.
  • Ability to perform Start of Care visits, evaluations, recertifications, and treatment visits in home settings.
  • Reliable transportation for patient visits. Mileage reimbursement provided between visits.
  • Preferred: Lymphedema therapy experience.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days 0800-1600. 5x8 schedule.
  • Weekly Hours: 40 hour guarantee.
  • Coverage Areas: Primary coverage includes Pickaway, Fairfield, Ross, Muskingum, Licking, Perry and Hocking counties as assigned.
  • Documentation Platform: HCHB (HomeCare HomeBase).
  • On Call/Weekend Requirement: None.
  • Dress Code: Scrubs; no specific color required.
