Minimum Requirements
- Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Washington State Physical Therapist license is required to start the assignment.
- BLS/CPR certification required.
- Home health experience required. Experience with SOC, evaluations, ROC, and recertifications preferred.
- Experience with HCHB electronic medical record preferred.
- Ability to cover patients within a 50 mile radius of the primary office location.
- Ability to complete weekly 40 hours as scheduled.
- Business casual dress required.

- Facility type: Home Health.
- Shift name: Days.
- Shift details: Days 0800-1600 (schedule noted as Days 8:00 am - 4:30 pm in client notes).
- Weekly guaranteed hours: 40.
- On call/weekend requirement: No.
- Coverage area: 50 mile radius; mileage reimbursed between visits from first to last patient.
- Primary responsibilities: Start of Care (SOC), evaluations, return of care (ROC), recertifications, and subsequent visits.
