Minimum Qualifications
- Doctor of Physical Therapy (DPT) degree required if licensure was obtained on or after January 2016; or foreign equivalent.
- Active Physical Therapy license in the state of practice required.
- Minimum 1 year of home health physical therapy experience required.
- Must have knowledge and experience with HCHB and OASIS.
- Valid Driver‘s License and at least 21 years of age required.
- AHA BLS or AHA-CPR certification required.
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support certification required.
Preferred Qualifications
- Experience working in home health settings beyond one year preferred.
Assignment Details
- Shift Days: Monday through Friday.
- Schedule: 5 x 8s, no weekends, no call.
- Must be able to travel locally and drive to patient homes. Mileage paid per government rate.

Impacting Patient Care
Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions while meeting department objectives. Serve as co-leader in an interdisciplinary approach to patient groups. Provide home health physical therapy services with documentation in HCHB and completion of OASIS.
