Minimum Qualifications

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois Physical Therapy license required to start the assignment. Minimum education requirement: Bachelor's degree. BLS certification from AHA or ARC may be required. Experience or ability to perform in-home physical therapy evaluations and interventions. Client-specific requirement: ability to work coverage across Williamson, Jackson, and Franklin counties.

Preferred Qualifications

Home health experience preferred. Experience with OASIS documentation preferred. Experience with Homecare Homebase (HCHB) preferred.

Assignment Details

Shift hours: 2000-0400. Weekly hour guarantee: 32 hours. On-call/weekend requirement: No. Mileage reimbursed between patient visits, from first patient to last patient. Coverage areas include Williamson, Jackson, and Franklin counties.
