Key Responsibilities

Collaborate to provide management and delivery of patient care plans Support patients in Lebanon, PA and surrounding areas Meet critical deadlines for charting Self-manage time and schedule effectively

Travel

Ability to spend approximately 20% of the day driving to/from patient locations Schedule: Monday-Friday, 8am-5pm *no on-call or weekends

Qualifications Required

Associate degree or higher 1+ years of experience in ANY of the following: Home Health Care Home Health 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Practice Partner EMR EHR Implementation Valid Regular Driver License (DL) in the state of Pennsylvania (PA) Valid Physical Therapy Assistant in the state of Pennsylvania (PA) Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months? (no) Are you okay with the required travel for this position? (yes)
