Physician Family Practice-Without OB - Competitive Salary

Opportunity:

Physician - Family Medicine - Geriatric Medicine, Internal Medicine - Geriatric Medicine

Job Details:

• Outpatient, clinic-only position with no call requirements


• Monday through Friday schedule, with consideration for 3-4 days per week


• Shift hours are 8:00 AM to 5:00 PM with 8 guaranteed hours per day


• Minimum of 2 weeks per month required, with full-time preferred


• See 4-6 patients per shift, serving exclusively adults over the age of 55 through a PACE (Program of All Inclusive Care for the Elderly) program


• Provide both in-clinic visits and community-based visits at patient residences, including assisted living facilities, skilled nursing facilities, and personal homes


• Off-site visits involve driving to patient locations and are typically accompanied by a nurse from the care team


• Work alongside an 11-person interdisciplinary team with most patient management coordinated through the EMR


• Geriatric experience strongly preferred as the majority of the patient population is elderly


• Board certification or true board eligibility required


• Active state license and DEA required


• BLS certification required

This facility operates as a PACE (Program of All Inclusive Care for the Elderly) program, dedicated to providing comprehensive, all-inclusive care to elderly adults aged 55 and older. The program utilizes a collaborative, interdisciplinary team model to coordinate and deliver high-quality medical care both within the clinic and in patients' home and community settings.
