Physician Lead, Geriatric Primary Care

HarmonyCares seeks a Physician Lead, Geriatric Primary Care to deliver and oversee home-based primary care for medically complex, home-limited patients in Fulton County, GA. In this role, you will provide direct geriatric primary care, guide an interdisciplinary team, and coordinate in-home visits, telehealth, and care management to reduce avoidable hospitalizations. You will mentor clinical staff, champion patient-centered, evidence-based care, and partner with community providers.
