What You'll Be Doing
- Coordinate care and transitions for acute care patients
- Collaborate with interdisciplinary teams to ensure timely patient progression
- Conduct initial and ongoing assessments to determine patient needs
- Develop comprehensive care coordination plans
- Monitor length of stay and manage transitions to appropriate care levels
- Document patient interactions and care plans in electronic health records
What We Need To See
- Current CA Registered Nurse license
- BLS certification
- At least 5 years of RN Case Management experience
- Inpatient Acute Case Management experience
