The Role:
• Specialty: OB/GYN (General OB/GYN coverage)
• Location: Fort Morgan, CO
• Assignment start: ASAP (Ongoing dates begin when schedules are released; ongoing availability expected June onward)
• Assignment type: Travel Locum
• Number of positions: 1
• Shift Type: 24-hour call (7a-7a)
• Typical monthly shifts: ~10-15 shifts per month for all locums ongoing
• Cases/Responsibilities: Hospital call - general OB/GYN, deliveries as needed (approximately 15 deliveries/month)
• Support staff: RN
• Setting: Call; No trauma / Not rated
• EMR: Epic (modules and virtual EMR training required prior to first day)
• On-call: Yes (weekend coverage expected)
• FTE: 1
• Emergency privileges: Yes, with clean file

Credentialing & EMR Requirements:
• Credentialing timeframe: Standard 60 days; 30 days or less possible for temps
• Pre-application and full application must be submitted within 72 hours of receipt of initial app to allow AD account creation for EMR access.
• Self-query NPDB report (dated within 30 days of presentation) is mandatory at time of submission.
• Once moved to "applicant in training," AD account creation begins (can take up to 5 days).
• EMR Modules must be completed prior to virtual EMR (PPL) training. Virtual EMR training can only be attended if Modules are complete and must be finished at least one week prior to the provider's first day.
• Candidates must complete the EMR questionnaire at time of submission (Epic experience, last use date, last facility used Epic, timezone for training, best phone & email, and a badge-quality photo uploaded to documents).

Schedule / Availability Specifics:
• All stints end the following morning at 7a.
• Example upcoming dates: July 15; July 17-20 (Ongoing dates begin June when schedules are released).
• Weekend call and weekday call expectations apply; average worked callback hours (for planning): Weekday ~6.88 hours; Weekend ~5 hours.

Travel & Lodging Preferences:
• Preferred lodging (when available): Fairfield Inn & Suites by Marriott, Fort Morgan or Hampton Inn, Fort Morgan - book at corporate rate. Other lodging sites will not be approved.
• Air travel acceptable; preference will be given to local candidates.
• Any anticipated travel exceptions (e.g., dietary restrictions impacting lodging) must be detailed in the provider presentation; requests without this information may be denied.

Submission Requirements (Must Haves):
• Board Certified OB/GYN or Truly Board Eligible (0-5 years post-residency) - REQUIRED
• Active Colorado license or active IMLC with LOQ in hand at time of submission - REQUIRED
• Self-query NPDB report (dated within 30 days) - REQUIRED at presentation
• Acknowledge and agree to complete EMR training in the required order - REQUIRED
• Acknowledge requirement to stay at preferred hotel when rooms are available - REQUIRED
• Candidate must be available for the specified stints to be considered for ongoing dates - REQUIRED
• Candidates must specify how many shifts per month they can offer ongoing - REQUIRED
• BLS and DEA are highly preferred and will be required during credentialing; transferred DEA may be considered but must be retained for duration of assignment - HIGHLY PREFERRED
• Clean medical/professional background and documentation - HIGHLY PREFERRED
