* Openings: 1DescriptionAmerican Traveler is seeking an experienced RN for a Home Health/Hospice position requiring at least 1 year of experience and a WI or compact RN license, serving a primarily geriatric patient population.Details
* Home health and hospice care setting (post-acute, community-based)
* Primarily geriatric patient population with occasional pediatric cases; common diagnoses include CHF, COPD, pneumonia, dementia/Alzheimer's, stroke, cancer, diabetes, complex wounds, and post-surgical recovery
* Nurse will conduct both home health and hospice visits - no floating to other departments
* Caseload of 5-6 patients per day, approximately 25-30 points per week, including routine and OASIS visits
* CPAP and BiPAP equipment encountered on occasion; patients self-manage - no ventilators
* Charting system: Cerner (transitioning to Epic)
* Monday-Friday, 8:00am-4:30pm (end time may vary); 8-hour shifts
* On-call required 2 times per week, Monday-Thursday
* Weekend work required only during on-call shiftsRequirements
* Active WI RN license or compact RN license (must be active by start date)
* Current BLS certification (AHA)
* Minimum 1 year of RN experience required
* Home health or hospice experience strongly preferred
* Required skills: chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, and complex wound care
* Hospice-specific skills: pain and symptom management, end-of-life care, empathy, and compassion
* Strong critical thinking abilities required
* Travel nursing experience preferred
* 2 professional references required: 1 supervisor reference from within the last 12 months of relevant experience, and 1 peer or supervisor reference from within the last 3 yearsAdditional Information
* Conduct home and hospice visits including routine assessments and OASIS evaluations
* Participate in weekly interdisciplinary team (IDT) meetings, case conferences at the office, and monthly all-staff meetings
* Supported by 3 certified nursing assistants; administrator is available after hours, weekends, and holidays
* Orientation includes 1 day of computer-based training plus 2-5 days of field training/shadowing depending on experience
* Solid-color scrubs required (no patterns or logos unless company-issued); closed-toe, non-slip shoes with socks or hosiery required
* Candidates who were previously permanent staff must have an end date at least 6 months prior to their application date
