Admissions Registered Nurse - Westchester County

WCS Healthcare Partners is seeking a dedicated and compassionate full-time Admissions Registered Nurse (RN) for a reputable skilled nursing facility in Westchester County. The Admissions RN is integral in managing new resident admissions, ensuring a seamless transition into the community, and delivering exceptional, resident-centered care.

Responsibilities

• Coordinate and oversee all aspects of new resident admissions and intake, including full body checks and wound care.
• Conduct thorough nursing assessments and initiate individualized care plans.
• Provide direct nursing care as needed, ensuring resident safety and comfort.
• Supervise and delegate tasks to Nursing Assistants in accordance with facility standards.
• Accurately document assessments, care plans, and progress notes.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor resident conditions and promptly report changes to appropriate clinical staff.
• Ensure compliance with infection control, safety protocols, and regulatory standards.
• Collaborate with interdisciplinary team members to support resident health and well-being.
• Uphold professionalism and embody the mission, vision, and values of WCS Healthcare Partners.

Requirements

• Active NY State RN license and current CPR certification required.
• Prior experience in admissions, long-term care (LTC), or skilled nursing preferred.
• Strong clinical assessment, documentation, and organizational skills.
• Excellent communication and interpersonal abilities.
• Ability to manage multiple priorities and work effectively within a team.
• Knowledge of regulatory and compliance requirements for long-term care facilities.
• Experience in long-term care (LTC) preferred; qualified candidates without LTC may be considered.
• Skills in wound care required.
• RNs: IV administration required.
