The Occupational Therapist (OT) is responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions that maximize independence, function, and quality of life. The OT collaborates with physicians, nursing staff, therapy professionals, residents, and families to achieve optimal rehabilitation outcomes.
Essential Responsibilities- Perform comprehensive occupational therapy evaluations.
- Develop and implement individualized treatment plans based on resident needs and goals.
- Provide skilled occupational therapy services to residents recovering from illness, injury, surgery, or functional decline.
- Assess and recommend adaptive equipment, assistive devices, and environmental modifications.
- Educate residents, families, and caregivers on therapeutic techniques and safety practices.
- Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
- Participate in interdisciplinary care plan meetings.
- Monitor resident progress and modify treatment plans as appropriate.
- Ensure compliance with all regulatory and professional standards.
- Current Colorado Occupational Therapist license in good standing.
- Skilled Nursing Facility, rehabilitation, or long-term care experience preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within a multidisciplinary team.
