Minimum Requirements
- Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. (Client requires Doctorate Physical Therapy.)
- Active Oregon Physical Therapy license required to start the assignment.
- Home Health experience required. (HH EXPERIENCE REQUIRED.)
- Experience with HomeCare Homebase EMR preferred.
- BLS certification from AHA or ARC may be required.
- Ability to perform home health PT duties: SOC, PT evaluation, recertification, ROC, and supervisory visits.
- Willingness to travel within Jackson County for patient visits. Mileage reimbursed between visits from first patient to last patient.
Preferred Qualifications
- Prior home health travel experience.
- Familiarity with productivity expectations and point-based productivity systems.
- Comfort with business casual dress or any color of scrubs.
Assignment Details
- Shift hours: 0800–1600 (client notes shift schedule Mon–Fri 0800–1630 and Days 0800–1600).
- Workweek: Monday through Friday.
- Coverage area: Jackson County, OR.
- Weekly hour guarantee: 40 hours.
