Physical Therapist - Travel Contract

Minimum Requirements

• Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. (Client requires Doctorate Physical Therapy.)
• Active Oregon Physical Therapy license required to start the assignment.
• Home Health experience required. (HH EXPERIENCE REQUIRED.)
• Experience with HomeCare Homebase EMR preferred.
• BLS certification from AHA or ARC may be required.
• Ability to perform home health PT duties: SOC, PT evaluation, recertification, ROC, and supervisory visits.
• Willingness to travel within Jackson County for patient visits. Mileage reimbursed between visits from first patient to last patient.

Preferred Qualifications

• Prior home health travel experience.
• Familiarity with productivity expectations and point-based productivity systems.
• Comfort with business casual dress or any color of scrubs.

Assignment Details

• Shift hours: 0800–1600 (client notes shift schedule Mon–Fri 0800–1630 and Days 0800–1600).
• Workweek: Monday through Friday.
• Coverage area: Jackson County, OR.
• Weekly hour guarantee: 40 hours.
