Quick Facts

- Start Date: Immediate or upon credentialing (~4 months)
- Position Duration: Permanent
- Shift: Monday–Friday, 8:00 A.M. – 5:00 P.M.
- Hours/week: 40
- Employment Type: Full-time
- Licenses/Certifications: Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience: At least 1 year post-training outpatient geriatric experience
- Pay rate: $1.00/hour (discussed upon application)
- Benefits: Pay package discussed upon application
- Additional Perks: No travel reimbursement; local candidates preferred
- Provide comprehensive outpatient geriatric evaluations and management
- Manage chronic and complex health conditions of elderly patients
- Conduct preventative care and health screenings
- Document patient encounters accurately using Epic EMR
- Collaborate with a multidisciplinary team to optimize patient outcomes
- Participate in call coverage (approximately one week per year), including after-hours and weekends
- Educate patients and families on health conditions and treatment plans
- Active Board Certified or Eligible in Internal Medicine or Family Medicine
- Minimum 1 year of post-training outpatient experience
- Strong geriatric care skills and comfort managing complex conditions
- Excellent documentation skills and familiarity with Epic EMR
- Ability to work independently in a clinic-based environment
