Quick Facts
- Location: Portland, OR (4400 Northeast Halsey Street)
- Start Date: 09/26/2026
- Schedule: Monday–Friday, 8am–5pm (considering 3–4 days/week)
- Duration: Ongoing, minimum 2 weeks per month
- Shift: 8 hours (flexible for full-time, potential for overtime)
- Hours/Week: 40+
- Employment Type: Contract
- Licenses/Certifications: Active Medical License, DEA, BLS
- Experience: Geriatric care experience preferred, minimum 2 years in outpatient setting
- Hourly Pay Rate: $1.00/hour

- Provide outpatient primary care services to adults aged 55+ with a focus on geriatric medicine
- Conduct community and in-home visits, including driving to patient residences in assisted living, SNFs, or private homes
- Collaborate with an 11-member interdisciplinary team to coordinate patient care
- Manage patient conditions through EMR documentation and team communication
- Participate in orientation and ensure compliance with licensing and certification requirements
- Support patients with chronic disease management and preventive care strategies
- Active medical license in Oregon
- DEA registration
- BLS certification
- Geriatric or outpatient primary care experience (minimum 2 years preferred)
- Strong communication skills and patient-centered approach
