RN - Regional Field Nurse Supervisor
Apply NowWCS Healthcare Partners is seeking a Regional Field Nurse Supervisor to support a large social services agency serving patients throughout Bronx and Manhattan, NY. This role provides clinical oversight, quality monitoring, staff supervision, and direct nursing care within patients' homes and designated clinical settings.
Job Responsibilities
- Conduct joint supervisory field visits with clinical field staff to assess competencies and provide clinical oversight.
- Provide training, in-service education, supervision, and guidance to Clinical Health Nurses (CHNs), Personal Care Aides, and Home Health Aides.
- Audit and validate clinical documentation to ensure accuracy, consistency, regulatory compliance, and adherence to evidence-based nursing practices.
- Provide clinical leadership and operational oversight for assigned cases, including coordinating care activities, monitoring service delivery, and ensuring quality standards are met.
- Review and monitor clinical documentation, visit completion, and follow-up activities to ensure compliance with organizational and regulatory requirements.
- Identify trends, barriers, and gaps in care delivery and collaborate with interdisciplinary team members to implement solutions that improve patient outcomes.
- Collaborate with partner organizations, care coordinators, and interdisciplinary teams to meet patient needs and ensure continuity of care.
- Perform routine home visits, including wound care, medication administration and reconciliation, and general health and functional assessments.
- Assess patient conditions and changes in health status and communicate updates to the care team and physicians as needed.
- Provide education to patients and caregivers regarding disease management, medication adherence, wound care, and home safety.
- Participate in interdisciplinary case conferences and clinical meetings as needed.
- Perform nursing duties within the Diagnostic & Treatment Center (DTC) or medical practice in accordance with organizational policies and professional scope of practice.
- Support the Care Delivery Team as needed through cross-training for the Care Delivery Manager role.
- Travel between patient homes and other locations throughout the designated service area.
Minimum Qualifications
- Current, active, and unrestricted New York State RN License.
- Graduate of an accredited school of nursing.
- Completed NYSED-approved infection control coursework.
- Minimum 3 years of clinical nursing experience, with 1–2 years of home care or a similar healthcare setting preferred.
- At least 1 year of experience working with a frail or elderly population preferred. Candidates with less than one year may be considered if they meet all other requirements and complete appropriate PACE-provided training upon hire.
- Strong clinical assessment, critical thinking, communication, and organizational skills.
- Proficiency with computers and electronic health record (EHR) systems.
- Experience with long-term care or managed care systems preferred.
- Valid New York State driver's license and access to a vehicle required.
- Ability to travel between patient homes and work locations throughout the assigned service area.
- Must be available for weekend, nighttime, and on-call coverage as needed.
