Community Behavioral Health Support Specialist

Provides community-based behavioral health assessment, treatment planning, crisis response, case management and skills support for individuals experiencing behavioral health concerns. Coordinates care with clients, families, medical staff and community agencies, documents services, and advocates for clients’ needs and preferences.
