Home Health Physical Therapist

Provide skilled physical therapy to patients in their homes, helping them improve mobility, safety, independence, and quality of life. Evaluate patients, develop and adjust treatment plans, coordinate care with interdisciplinary teams, educate patients and caregivers, and document services. The role serves a geographically assigned area and includes travel to patients’ homes.
