Position Overview We are seeking a dedicated and licensed Physical Health Therapist to join our team at Top Acres Behavioral and Psychological Center. Work Environment This role is based onsite at our well-equipped rehabilitation center in Pomona. Regular business hours are maintained, with occasional evening shifts during peak times to accommodate our patients' needs. Key Responsibilities * Conduct comprehensive evaluations of patients' physical health needs through assessments, screenings, and patient interviews to determine individualized treatment plans. * Implement evidence-based therapeutic interventions, including physical training, therapeutic exercises, and modalities for pain management. * Monitor patient progress through regular assessments and adjust treatment plans as necessary to ensure optimal recovery. * Collaborate closely with physicians, occupational therapists, and other healthcare team members to create holistic, patient-centered care plans. * Educate patients and their families on diagnosis, treatment options, and home exercise programs to promote ongoing rehabilitation and independence. * Participate in departmental meetings and peer supervision to foster a culture of continual learning and best practice sharing. Patient Population / Client Focus Our patients range from those recovering from surgeries, serious injuries, chronic pain conditions to those requiring rehabilitation after neurological events. The diverse patient population you will serve includes individuals of varying ages and backgrounds, each with unique rehabilitation requirements. Your role will directly impact their quality of life, helping them to regain mobility, strength, and confidence in their daily activities. Qualifications * Must hold an active state license as a Physical Therapist. * A Bachelor's Degree in Physical Therapy or a related field is required, with a strong emphasis on clinical expertise and patient care. * Proven experience working in a rehabilitation setting is preferred. * Exceptional communication skills to interact effectively with patients, families, and medical staff. * Strong analytical skills for evaluating patient progress and making necessary therapy adjustments. Preferred Experience * Experience in a rehabilitation center or similar healthcare setting is advantageous, particularly working with patients recovering from orthopedic or neurological-related conditions. * Familiarity with electronic health record (EHR) systems for documentation and tracking patient progress is desirable. * Specialized training in manual therapy techniques or sports rehabilitation is a plus.
